Membership Rates
Currently, all visits are provided via telemedicine, allowing you to receive care from the comfort and convenience of your home.
In-person appointments are planned for the future as Elevate Wellness grows — bringing even more connection and accessibility to your care experience.

Individual: $99/month

Children (ages 17 &under): $49/month

Families (up to 6)*: $250/month

Telemedicine Only: $50/month

30-day Telemedicine Access**: $200 due at time of enrollment. No enrollment fee. Limited to one 30-day trial per lifetime.
Your membership includes everything you need — even hormone therapy and weight management.
Because at Elevate Wellness, these aren’t “extras.” They’re part of your health.

One-time Enrollment Fee (for memberships): $100 for individuals or $200 per family

*Family constitutes the member plus 2 or more (spouse/children) of the same immediate family up to 6 members. Additional fee of $25 per member for families of 7 or more.
**This gives those that are unfamiliar with the Direct Primary Care model a chance to see what it's all about! You won't want to go back to the traditional system, we promise. If you decide to continue enroll in a membership immediately following the 30-day Telemedicine Access, your one time enrollment fee will be waived. This is for single individuals only. There are no family plans available for 30-day Access. Any future visits require enrollment in a membership plan.
